Welcome to Empower Yoga and Wellness Yoga as Therapy.
Before our first session, I'll need to gather some information from you, as well as provide you with an overview of my professional practices.
FORMS
Below you'll find a link to a Therapist-Client Agreement, by which you and I will establish a "frame" for our treatment. It's important for both the health of our therapeutic alliance - as well as the quality of your treatment - that this frame be well-constructed, and mutually agreed upon. Please review, and let me know if you have any questions. Please return a signed copy to me in anticipation of our first session. I will return a signed copy to you with your first plan of care.
Additionally, below, you'll find a link to a New Client Questionnaire. Please download, fill out, and return before we meet for the first time.
Lastly, I may ask you to complete a Scale of Body Connection. This is a clinical scale used to assess your relationship to your felt experience. It’s a good tool to help us start our conversations around your relationship to yourself, body and being. You’ll find it a the bottom of the page; please, too, fill it out and return it with the rest of the forms.
PAYMENT & CANCELLATION POLICY
A few important points regarding the structure of our sessions:
Initial sessions are 90 minutes at a rate of $150; follow-up sessions are 55 minutes at a rate of $120.
Sessions must be paid, in-full, at the beginning of each session. Payment can be made via Paypal to julia@empoweryogatherapy.com, or Venmo to @empower-yoga-therapy. Payments via check and Zelle can also be arranged.
All sessions not cancelled with 24 hour notice will be charged the full session rate.
Let me know if you have any questions. Yoga Therapy is a process. Essential to that process is the therapeutic alliance, the foundation for which is clear, whole-hearted communication.
